1. Length of Service
Have you been employed by your employer for at least 12 months of continuous service?
2. Reason for Dismissal
Was your dismissal related to any of the following?

- Health & Safety: Reporting a safety issue at work
- Statutory Rights: Asking for your legal entitlements
- Family: Pregnancy, Maternity, or Paternity leave
- Whistleblowing: Reporting illegal activity
